Our practice area covers a 3 mile radius from Hoole Lane, Boughton which includes: Boughton, Vicars Cross and Hoole Lane.

Anyone who lives within our practice area can register with the practice.

Please refer to the map opposite for more detail.

If you are unsure about whether we can register you please call us on 01244 325421. It will be helpful if you can provide your postcode.

• All new patients are offered a new patient check with our practice nurse.
  
• You will need to come into the surgery to complete the registration form and a new patient questionnaire.

To help save you time when registering you can download our new patient questionnaire from our downloads section. Print the questionnaire, complete it and bring it along with you.

Please also remember to bring your Medical Card with you - this will save you even more form filling time.

Any person requesting to be registered as a patient will be asked to provide proof of identity and address.

If you are unable to provide such proof, we will not be able to register you.

Below is a list of items that will be accepted. You will be required to bring with you one proof of identity that includes a photograph and one proof of identity that includes proof of address:

• Birth Certificate
• Marriage certificate
• Medical card
• Driving license
• Passport
• Local authority rent card
• Paid utility bill
• Bank/building society card statement
• National insurance number card
• Payslip
• Letter from benefits Agency/Benefits book/signing on card
• Papers from the home office
• P45

The following documents will not be accepted as proof of identity: 

• Library card
• Video rental card
• Health club card
• Private rent book

We will also need to know the following:

• Are you already registered in Chester? If so where.
  
• Which area you are moving from.
  
• Details of your current GP.
  
• The details of family members/dependants who are also registering here.
