New Patient Registration
New patients registering at the surgery will only be eligible if they live within a 3 mile travelling distance, excluding postcodes CH4 7** and CH4 8**. Existing patients that move house more than a quarter of a mile beyond our 3 mile limit will be asked to re-register at a new surgery. If the address is in Wales, we are not able to accept the registration due to different healthcare, IT systems, and administrative arrangements in operation there.
The three mile travelling limit will be determined by use of new technology e.g. “Google Maps”. We ask all new patients to complete two simple forms. This information is available to download via the above "New Patients > Registering Online" tab. Below is a map of Boughton Health Centre's boundary; this map is for illustrative purposes only, and will not be used as a means of determining eligibility.
In addition, all new patients are required to produce 2 separate forms of identification; photographic evidence of ID and proof of address. If you are unable to provide such proof, we will not be able to register you. If you are receiving on-going treatment please bring all the details with you.
Medical treatment is available from the date of registration. Please contact reception for further information.
Below is a list of items that will be accepted. You will be required to bring with you one proof of identity that includes a photograph and one proof of identity that includes proof of address:
- Birth Certificate
- Marriage certificate
- Medical card
- Driving license
- Local authority rent card
- Paid utility bill
- Bank/building society card statement
- National insurance number card
- Letter from benefits Agency/Benefits book/signing on card
- Papers from the home office
The following documents will not be accepted as proof of identity:
- Library card
- Video rental card
- Health club card
- Private rent book
We will also need to know the following:
- Are you already registered in Chester? If so, where?
- Which area you are moving from.
- Details of your current GP.
- The details of family members/dependents who are also registering here.
Electronic Transfer of Records
If you have previously registered with a different GP in England, upon registering at this practice your electronic health record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.
