Patient (EMIS) Access

Patient Access is a secure Internet system whereby patients can perform, on-line, certain tasks usually carried out over the telephone or in person.

If you register to use Patient Access without getting your details from the practice, you will have limited access to the available services. Please note that you must visit the practice to confirm your registration within 1 month otherwise your application will be rejected and you will have to re-register for the service.

As we can only offer Patient Access to patients aged 16 and over we can allow you to book an appointment in your name for your child. You MUST change the name to your childs name at the reception desk when you check in. PLEASE NOTE - 1 appointment is for 1 patient. A clinician can only see 1 patient per appointment

To confirm your registration, 2 forms of documentation must be provided as evidence of identity and one of these must contain a photo.

Acceptable documents include: 

  • Birth Certificate
  • Marriage certificate
  • Medical card
  • Driving license
  • Passport
  • Local authority rent card
  • Paid utility bill
  • Bank/building society card statement
  • National insurance number card
  • Payslip
  • Letter from benefits Agency/Benefits book/signing on card
  • Papers from the home office
  • P45
