When the surgery is closed please
dial 01244 343300 to contact DEMS (Deva
Emergency Service) for a duty doctor. Please restrict these
calls to genuine emergency problems.
Firstly you must have visited the surgery and signed up to the service's
Terms & Conditions.
You will have then received a Registration letter in the post. This
letter will contain a box with details similar to the following:
REMEMBER to note your Access ID and our Practice ID (always
2810) and keep them safe; just as you would your bank PIN number and
online banking details.
You will need your Access ID and our Practice ID to log on in
future.
You now need to activate your account using the details contained
within the registration letter. At this stage you need to visit our
homepage and
click on the "Activate (new users)" link or click on the
link on this page.
This will take you to an external secure logon website that looks
similar to the image below (if this does not happen and the page fails
to load try again
later).
Using the information given
in the same four boxes of the section "Registration Screen 1" of
you registration letter complete the boxes on the webpage. Ensure
you type the information exactly as it
is shown on the letter. Then press the 'Submit' button.
You will now be taken to "Registration Screen 2" which will look
similar to the image below:
You may be asked for either your Surname or Firstname for security
reasons. Type them in as shown exactly by your registration letter.
Complete all
other
details;
including setting a password and your personal security question.
That is it, congratulations! You are now registered and are able
to log onto the system each time using the Practice ID, Access ID and
password
combination
(see screen shot below):
